Parent Consent for iCivics

College Station ISD, in compliance with state and federal laws and regulations, requires parent/guardian consent before creating accounts on behalf of students.

Your student’s [subject] teacher would like to use [application name], an online learning tool that students will use to [brief description].

For students to use this program/service, certain information must be provided to the application provider. Under state and federal laws, these websites are required to provide parental notification and obtain parental consent before collecting personal information from students.

Schools are permitted to collect parental consent for the collection of personal information on behalf of the program/service, thereby eliminating the need for individual parental consent to be given directly to the website operator. To do so, the District is providing you with this notice allowing you to opt-in.

CSISD is a member of the Texas chapter of the Student Privacy Consortium (Texas Student Privacy Alliance). We require all programs/services that collect student data to have a signed data privacy agreement with us through the consortium. After reviewing this site, if you agree for your student to use (Type Website Name Here) in class, please complete the attached form and return to school by (Date) If we do not receive your consent form on or before (Date), we will not create an account for your student to use with (Type Website Name Here).

If you agree to allow your student(s) to have an account with this program/service, please complete the form below. If you choose not to opt-in, no action will be taken, and no account will be created for your student.

Please submit one form per student ID number. You will receive a confirmation email when your submission has been received. If you don’t see the confirmation email, please check your spam folder.

You will not be able to submit more than once per student ID number. Your confirmation email will provide instructions on how to correct any errors, if necessary.
